
In families’ own homes, we offer a seamless service of care for children and young people, age 0-19 years; working closely with a variety of care providers to provide cancer care, palliative care, end of life care and a wide range of other care and support services.
Each patient is assigned to a named ‘chYps’ Specialist Nurse who, with their colleagues, will provide the clinical in-home Hospice Care; liaise with other health and social care services; and help the patient and family access other relevant ‘chYps’ services.
These services include Music and Play Therapy, Counselling, Chaplaincy and pre and post bereavement support for the whole family.
A Short Break is fun and therapeutic for the child, or the siblings, and can provide respite for the parents.
We offer a variety of Patient and Family Support and Social Groups: a Youth Group; a Transition Youth Group; siblings groups; Coffee mornings; Music Therapy Groups; and a Parents’ Bereavement Group.
The care we provide is available 24 hours a day, 7 days a week and 365 days a year.
In the event of urgent issues, all families have access to our 24 hour on-call service.
Our services are provided completely free of charge to children, young people and their families.
Please note that in Mid and South West Kent, our service is currently restricted to end of life care. However, we are actively raising the funds to enable us to progressively introduce our full service as outlined above.
